b'PRIMARY CARE: Community ImpactDebbie McLaughlinRN, MS, BSN I PRACTICE ADMINISTRATION, DIRECTOR PRIMARY CAREThroughout the many challenges of the COVID-19 pandemic, primary care nurses, medical assistants and other team members have made it a priority to make available and administer COVID-19 vaccines to our patients in the Primary Care setting. After holding several centralized COVID-19 vaccine events for Concord Hospital Medical Group (CHMG) patients in 2021, we heard from patients that they would like the opportunity to receive COVID-19 vaccine in their own primary care practice. They cited both convenience and the trust they have in their primary care team. With the focus on the health and safety of our community and our patients, primary care nurses and medical assistants have administered more than 5,300 COVID-19 vaccines in our practices since March 2022.All CHMG-Concord Primary Care clinical teams and Belmont Pediatrics:Worked with the State of New Hampshire Department of Health and Human Services to become approvedCOVID-19 vaccination sites; Completed clinical competency training for registered nurses (RNs) and Medical Assistants (MAs); Completed training on entering COVID-19 vaccine data into the New Hampshire Immunization Information System. We understand that you are trying to makethe best decisions you can about vaccines to keep yourself and your family safe.Lets talk about it!Were here to help.9/16/22 4:44 PMTotal COVID-19 Vaccine Administration at CHMG Primary Care Practices(March - November 2022)1000900 894EFM_VaccineMail_09_2022.indd 3Doses of COVID-19 Vaccine Administered (#)800 759 734700600 572 5845155004000311 301 310300200 188 1661000 0RICH FTH-War FHC-H FPP EFM PSFM FHC-C FTH-Hop PFP CFM IMED BPPrimary Care Practice12'
