Cerner clincial documentation of wound types and the Braden risk score were modified to generatea reminder for nurses to initiate a Pressure Injury Prevention order set and interventions. The number of new pressure injuriesand use of the nurse driven protocol for high risk patients is monitored monthly by the Clinical Practice Committee and Quality Performance and Patient Safety team.
