Nurses are collaborating to identify patients at highrisk for harm who would benefit from nurse-driven preventive measures. A data driven strategy is being used to roll out theseinitiatives across the health system.STEP 1 Analyze the data to understand where improvement is needed. STEP 2 Conduct research to understand what is the best, evidence-based practice for prevention.STEP 3 Create nurse-driven protocols to support evidence-based care and leverage the tools inCerner to automate these to the extent possible. STEP 4 Create access to actionable data and information for the direct care team. STEP 5 Develop unit-based workflows to leverage real-time patient safety information.
